About the Role As a RN Transfer Center Coordinator, you will be the critical link connecting patients to life-saving care across our facilities. Leveraging your clinical expertise, you will ensure smooth and efficient transfers , facilitate communication among providers, and contribute to the operational success of our patient logistics system. This team coordinates 800-1100 incoming patient transfers a month from all over the country and the world. What You'll Do- Coordinate Patient Transfers- Serve as the pivotal point of contact for managing patient transfers, prioritizing cases by acuity, and ensuring compliance with EMTALA guidelines.
- Be a Liaison and Advocate- Act as a trusted partner to referring facilities, UVA Health teams, and patients, ensuring a seamless transfer experience and delivering exceptional service.
- Prioritization and Triage: Apply advanced triage skills to assess and prioritize patient information, ensuring a clear and accurate clinical picture is communicated to accepting physicians.
- Leverage Cutting-Edge Technology- Utilize advanced communication systems, electronic health records such as EPIC, and patient flow tools to coordinate care with precision and efficiency.
- Provide Clinical Expertise- Perform telephonic triage, applying critical thinking to assess patient needs and determine the best course of action.
- Collaborate with UVA physicians and clinical teams to ensure patients receive appropriate care placement.
- Analyze and document patient data accurately for quality outcomes and operational improvements.
- Recognize critical and emergent situations and collaborate with referring and UVA physicians to determine appropriate care
- Data and Assessment Skills: Gather and document critical patient care data, including demographics, insurance information, and clinical details. Demonstrate expertise in assessing patient needs, documenting findings, and connecting this information to actionable interventions.
- Patient-Centered Focus: Assess health needs using established guidelines and clinical expertise, ensuring every patient is placed in the most suitable care setting. Advocate for patient well-being through informed decision-making and thorough follow-up.
- Access and Documentation: Facilitate patient access by collecting and verifying accurate demographic and financial data. Ensure all documentation is completed thoroughly and complies with organizational standards.
- Drive Operational Excellence- Identify opportunities to enhance processes, contribute to system-wide initiatives, and help shape protocols for better patient care.
- Process Improvement: Identify opportunities to streamline operations and enhance patient care pathways. Share insights and solutions with leadership to improve workflows and outcomes.
- Your Skills and Qualifications:
- Education: Graduate of an accredited RN nursing program and Associate's Degree in Nursing (ADN) required. Bachelor's in nursing (BSN) Preferred and Required within 5 years of hire date.
- Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of. Virginia required. A Valid Virginia Driver's License (VADL) may be required.
- Experience: Minimum of 3 years in Med/Surge bedside Nursing is required.
- 5 years as an RN, Critical Care, ICU or Emergency Room or experience in transfer coordinator role strongly preferred.
- Technology Skills: Proficiency with EPIC, PAS and SPOKE or similar IT systems is strongly preferred.

Position Compensation Range: $41.26 - $54.52 Hourly
