Description
Be the front door to health care. For many of our patients, a traditional clinic is the last place they'd walk into. So we moved the front door. About us: Central City Concern is a Portland-area nonprofit providing housing, healthcare, recovery, and employment services to people affected by homelessness. Our integrated model means your patients have support well beyond the exam room. Our Mobile Health program launched in 2023 to bring culturally responsive, trauma-informed, patient-centered care directly to people experiencing homelessness. In early 2024, two custom-built vans joined the effort, equipped for wound care, physical exams, infectious disease screening, vaccinations, and medications. We're looking for a physician, nurse practitioner, or physician assistant to help us do more of it. You are someone who has primary care or mobile medical experience and approaches every patient without judgment. You think about how decisions affect underserved communities, you practice with trauma-informed principles, and you're skilled at shared decision making and motivational interviewing. You're comfortable with de-escalation, or eager to learn it, and you work well in a multidisciplinary team. What your practice could look like:
- Prescribe medications for opioid use disorder without unnecessary hurdles between your patient and treatment.
- Harm reduction as a clinical approach. Meet patients where they are, build trust over time, and measure success by engagement and resilience.
- Whole-person medicine. Treat physical health, mental health, and substance use disorders together, the way your patients experience them.
- A team around you. Work alongside pharmacists, nurses, and programs for Hepatitis C treatment, withdrawal management, residential recovery, specialty mental health, and overdose prevention. You'll practice independently, with medical and administrative support on site and off.
- A culture of learning. Shape clinical workflows, contribute to quality improvement, and teach and onboard colleagues.
If you want your clinical skills to reach the people who need them most, we'd love to meet you.
