Summary: UNC Hospitals Department of Pharmacy has an immediate opening for a highly motivated pharmacist interested in joining our Nephrology Clinic in Chapel Hill. The successfulClinicalPharmacistPractitioner (CPP) will playanintegral roleinthe careofpatients within the UNCNephrology Clinic at the Eastowne Medical Office Building. This isamultidisciplinaryclinicwhichoffers comprehensive servicesinnephrology. TheNephrologyCPP will serveasanAdvanced Practice Provider (APP) on theinterdisciplinary health care teamand will provideclinical support to nephrologists, other APPs, nurses,and ancillary staff.Within this innovative care model, the CPP will support management of advanced chronic kidney disease (CKD) and associated complications (e.g. anemia, bone mineral disease), hypertension and use of guideline-directed medical therapy. As well as support and manage existing and emergingspecialty medications and biologics. The successful candidate will haveanopportunity to provide educational mentorshipand experiential precepting topharmacystudents, Post-graduate Year 1 (PGY1),and Post-graduate year 2 (PGY2)PharmacyresidentsatUNCHospitalsand work closely with medical residents and fellows. Additionally, theNephrologyCPP will work with theDepartmentofPharmacyand DivisionofNephrologytoaddressinstitutionalanddepartmental-related quality improvementinitiatives.Academicappointmentat theUNCEshelman SchoolofPharmacyisalsoavailable commensurate with experience. CompletionofaPGY2 specialty residencyinAmbulatory Care, Pharmacotherapy or other related specialized residency or fellowship oranequivalent levelofexperienceinclinical practice is preferred. Three to five yearsofpractice experienceinambulatory care would be strongly preferred. The designationofBoard CertificationinAmbulatory Care or Pharmacotherapyand/or prior experienceasaresidentand student preceptor is preferred.
