100% FTE. The scheduled shifts are Monday-Friday, 8 or 9-hour day shifts (10 hour shifts not available) between 7:30 am to 5:30 pm. No weekends. When onsite, you will work at the Administrative Office Building on UW Health Court, or the Northport Dr. Clinic, both located in Madison, WI. Pay:

Be part of something remarkable Join the innovative Home-Based Primary Care program working with the adult/geriatric population that are homebound. We are seeking a Registered Nurse (RN) to:
- Collaborate with a focused interdisciplinary team of MDs, APPs, Social Workers, Schedulers and other RN Care Coordination (RNCC) nurses to care for patients enrolled in the program.
- Perform the main duties of the RNCC nurse that include care coordination, triage and case management. Communication with patients/families is via telephone or video communication (no in-home visits at this time).
- Utilize your nursing skills such as triaging emergent issues, providing resources to clients and their families, initiate/facilitate consults/referrals, update care plans, and work with rehab/LTC facilities to coordinate care. It is preferred that nurses have 3-5 years of adult acute care or primary care nursing experience.

Qualifications
- Graduate of school of nursing Required
- Bachelor's Degree in Nursing or higher Preferred
Work Experience
- 6 months RN experience Required
- Previous experience in adult acute care or ambulatory setting Preferred
- Previous Case management experience Preferred
Licenses & Certifications
- Registration as a registered nurse in state where employed or licensure in the state licensure compact Upon Hire Required
- CPR/BLS Upon Hire Required
- Case Management Certification Preferred
