Resident Care Coordinator - LVN

Mon - Fri 6:30 am- 3:30 pm

Pay Range: $86,840 - $100,000

Summary:
Under the direction of the Executive Director/ Director of Health Services, the Resident Care Coordinator -LVN (Care Services Manager) is responsible for the coordination, supervision and directing of the work of the Assisted Living staff and ensures the resident needs are met according to Federal and State regulations as well as Company Policy.

• Directs the daily delivery of healthcare services at the community, ensuring resident care needs are appropriately assessed, addressed, and coordinated.
• Promotes a resident-centered approach to care by ensuring residents are treated with dignity, respect, compassion, and individualized attention as their needs evolve.
• Monitors care and service delivery across all shifts to maintain consistent standards of quality and responsiveness. Provides direct care support, including Caregiver and/or Med Tech to support and address resident needs or staffing requirements.
• Provides leadership and oversight of Caregivers, Med Techs and licensed nursing staff.
• Coordinates schedules and supports ongoing team member development and training through in-services, coaching, and situation-specific training.
• Oversees medication related processes to promote safe, consistent, and compliant practices, which may include but not limited to medication training, pharmacy coordination, medication supervision and/or administration and audits.
• Participates in the on-call rotation to support resident care, staffing, and operational needs as required.

Requirements:

• Registered Nurse - California or Licensed Vocational Nurse - California credential
• High School diploma or GED
• Experience in long-term care community health, geriatric and/or rehabilitative nursing desirable
• 5 years' progressive experience in Nursing, with at least 2 years of supervisory experience.
• Current BLS/CPR with First Aid certification required.
• Ability to read, analyze and interpret professional journals, technical procedures, or government regulations.
• Must be able to read, write, speak and understand English
• Basic mathematical skills required.
• Must be able to understand, follow, support and initiate policies and procedures of the facility. Must be able to recognize emergency situations and respond appropriately; must be able to plan, organize, prioritize and implement work assignments efficiently; must be able to function effectively with no supervision.

Preferred:
* 5 Years experience working in a senior community
* Long term care, geriatric and/or rehabilitative nurse
