Description Home Health and Hospice Nursing Are you a compassionate Registered Nurse who enjoys coordinating care, building relationships, and helping patients begin their home healthcare journey? Join theMaineHealth Care at Home Admitting Departmentas aPer Diem Registered Nurseand play a vital role in ensuring patients receive a seamless transition from hospital or provider referral to high-quality care in the comfort of their own homes. As an Admitting RN, you'll be the first clinical connection for patients entering home health services. Working closely with referral sources, physicians, care managers, and the interdisciplinary home health team, you'll complete comprehensive home care intake assessments, determine patient eligibility, coordinate admissions, develop individualized plans of care, and ensure patients and families understand the services available to support their recovery and independence. This role offers the opportunity to combine your clinical expertise with exceptional communication and care coordination skills. Every shift brings the chance to evaluate patients' medical needs, assess home safety, collaborate with providers, educate patients and caregivers, and ensure a smooth transition into home healthcare services. Compensation for part-time positions isdeterminedon a prorated basis consistent with the scheduled hours and applicable full-time equivalent (FTE).
