Location: Arlington Neighborhood ClinicDepartment: NHC Patient Fam Support SvcsShift: First Shift (United States of America)Standard Weekly Hours: 40Summary: The Family Advocate provides support and resources to families for issues that extend beyond their clinical needs. Builds individual and family capacity by increasing health knowledge and self-sufficiency. Responds to the social and emotional needs of our families, providing guidance and support while navigating them through the health care system and community to achieve the best health outcomes. Serve as both an advocate and liaison between the patient/family, health care team, and community partners. Functions in collaboration with the treatment team within the Cook Children's Neighborhood Clinics.
