Duties & Responsibilities

POPULATION SPECIFIC CARE
Neonate - <30 days

Adolescent - >13yrs to 18yrs
Adult - >18yrs to 65yrs

ESSENTIAL FUNCTIONS
An employee in this position may be called upon to do any or all the following essential functions. These examples do not include all the functions which the employee may be expected to perform.

1. Completes requirements of ASHP accredited residency program.
2. Coordinates, develops, and conducts major residency project, drug utilization review, and additional projects as assigned by pharmacy director, program director, and preceptors.
3. Fulfills service commitment by sustaining pharmacy services throughout the organization through appropriate medication preparation, dispensing, and inventory control. Provides oversight to pharmacy technicians, interns and students when providing coverage in assigned area.
4. Participates in the education of pharmacy staff, nursing staff, medical staff, pharmacy students, and patients and families. Participates on hospital committees and in drug policy development.
5. Provides drug information services by responding to questions regarding medication compatibilities, adverse drug events, regimens, dosing, interactions, storage and disposal, compounding, therapeutic uses of medications and pharmaceutical supplies in a timely manner; using a systematic approach to effectively searching, retrieving, and evaluating the literature and appropriately communicating and applying the information to the patient care situation; and assisting with the development, maintenance, and implementation of hospital and department policy and procedures related to medication use, storage, restrictions, and administration.
6. Sustains and implements quality and safety initiatives and practices in the department and throughout the hospital. Reports adverse drug events and medication variances using the hospital variance reporting program (QSRS).
7. Sustains clinical pharmacy services throughout the organization by providing pharmaceutical care for pediatric patients of all age ranges through participation in multi-disciplinary team rounds and/or care, emergency response, education, guidance, interpretation and analysis of medication related therapeutic issues; documenting pharmaceutical interventions according to departmental standards; performing therapeutic drug monitoring and disease outcome analysis; and timely verification of medication orders in an electronic health record system.
8. Participate in general pharmacy operations throughout the organization through appropriate medication preparation, dispensing, and inventory control. Provides oversight to pharmacy technicians, interns, residents and students as needed.

Minimum Qualifications

* Education: PharmD or RPh
* Experience: As defined by the ASHP residency standard.
* Licensure: Licensed pharmacist in the state of Colorado within 90 days of hire.
* Certification: BLS certification (American Heart Association, American Red Cross, or American Safety and Health Institute accepted)

Salary Information
Pay is dependent on applicant's relevant experience.

Annual Salary Range (Based on 40 hours worked per week): $54,790.77 to $82,186.16
