SUMMARY: Provide leadership and management of the Nursing and Health Education department. Manage existing health education and disease management programs. Coordinate with Nursing Leadership to develop and manage nursing education programs. Coordinate with RMC physicians and management, contracted health plans, and identified patients to develop new programs designed to improve patient quality of life and health outcomes through improved knowledge and treatment compliance.

Evaluates quality of patient care and patient outcomes as they relate to healthcare-associated infections; collects, prepares, and analyzes healthcare-associated infection data; presents infection data and makes recommendations for actions; monitors employee compliance in use of barriers and infection prevention measures; prepares and presents educational offerings for the staff; serves as a resource to all departments and personnel.

Qualifications

EDUCATION and/or EXPERIENCE: Bachelor's Degree in Nursing, a four-year college or university preferred. Minimum of 4 years of multi-facility supervisory experience in ambulatory care or other relevant healthcare experience, with operational oversight and budgeting experience. Must possess knowledge of current trends in nursing, patient health education and disease management programs. R.N. license required.

CERTIFICATES, LICENSES, AND REGISTRATIONS: Current California RN License; current BLS certification required.

ESSENTIAL FUNCTIONS:

1. Interact with appropriate RMC medical staff to ensure that educational programs are in accordance with practice standards and RMC Clinical Practice Guidelines.
2. Monitor existing educational programs for effectiveness and prepare reports for review by the Quality Risk Management Committee.
3. Conduct evaluations and surveys to ensure that health education programs meet the specific needs of the RMC population.
4. Document health education interventions in the medical record to keep physicians appraised of patient progress or lack of progress.
5. Assume responsibility for initiating a pleasant patient-oriented atmosphere by displaying a professional attitude through performance, appearance, and demeanor.
6. Select, orient, train, manage, and evaluate staff.
7. Nursing Education - Provide Clinical Skills training to clinical staff annually and during Nursing Orientation.
8. Establish work procedures, and write departmental policies, procedures, and guidelines. Keep departmental staff informed of any changes in policies or processes.
9. Prepare work schedules, maintain employee attendance records, and submit documentation for payroll.
10. Assess staff competencies. Coach and provide ongoing training and feedback to staff members.
11. Adhere to budgetary guidelines established by the Vice President, Value Based Care.
12. Communicate with the Vice President, Value Based Care regarding department operation, including problem solving within the department and recommend corrective action.
13. Regularly attend and participate in Health Education and QRM Departmental meetings.

14. Collaborate and partner with population health team, including medical directors, to meet and exceed quality related goals.
