Position Summary As a PACE LVN in California, the LVN provides basic, hands-on nursing care to frail older adults enrolled in PACE. Operating under the supervision of an RN, nursing manager, or provider, the LVN conducts data collection, implements care plans, administers medications, performs delegated nursing tasks, and communicates changes in participant status to the interdisciplinary team (IDT). The LVN's role is bounded by the scope of practice permitted by the California Board of Vocational Nursing and Psychiatric Technicians (BVNPT). Job Description Job Roles Clinical & Nursing Care
- Conduct basic assessment/data collection, such as obtaining participant histories, vital signs, and other routine observations, within the LVN license scope. Observing and documenting participant responses to interventions, treatments, and medications, and reporting significant changes to RN or provider.
- Following and contributing to the interdisciplinary plan of care.
- Providing patient interventions per care plan under RN or provider delegation - e.g. dressing changes, skin tests (e.g. TB), phlebotomy (if certified), non-medicated treatments.
- Assist with wound care: cleaning, dressing application, monitoring for signs of infection, but not performing advanced wound care / sharp debridement.
- Provides administrative support to the RN Case Manager, including securing outside office notes, prescription documentation, follow-up appointment information, and all necessary pre-procedure and pre-operative instructions.
- Providing patient education as directed by RN or a higher licensure.
- Keeps clinic workflow moving, focusing on productivity and efficiency, including assisting during lunch times and breaks, ensuring exam rooms are set, ordering supplies and vaccines to ensure correct supply is on hand, supporting provider and RN case manager productivity, and calling in refills to pharmacy.

- Attend all department meetings.
- Participates in regular performance reviews, in-service training, and continuing education to maintain compliance and competency.
- Performs all other duties as assigned and within scope of practice.
Medication Administration
- Administer prescribed medications per order: oral, topical, subcutaneous, intramuscular, and intradermal, as allowed in California.
- Perform blood withdrawal only if LVN holds a California board-approved certification.
- Administers skin tests and immunizations, with proper training and per supervising protocols.
- Document medication administration accurately in electronic health record, including patient consent, patient response, and escalating any adverse reactions or errors in a timely manner and per policy.
- Request for pharmacy refills of medication.
Compliance, Safety & Quality
- Practice in accordance with California vocational nursing regulations, the California Business & Professions Cole, and organizational policies.
- Consistently follows policies and procedures to ensure positive participant outcomes.
- Follow infection control, safety protocols, and PACE emergency procedures.
- Report incidents, adverse events, or scope-of-practice concerns to the nurse manager.
- Maintain required certifications (e.g. BLS) and license in good standing.
Requirements:
Current California LVN License. Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training. Traveling between sites and other locations is required as an essential function of the job. Must have a car, a valid California driver's license, and proof of minimum levels of car insurance as required under California law, although limits of $100,000 are recommended. An acceptable driving record is also required. California law requires all drivers to obtain a valid California driver's license within ten days of establishing residency. Mileage and other reimbursement governed by policy.
Experience/Specialized skills (including Language)
1 - 2 years of experience working with the older adult population preferred. Clinical competency with the older adult population preferred. Ability to work effectively in a team environment. Demonstrated ability to communicate effectively and openly with other interdisciplinary team members on participant status and needs. Must be able to multi-task, be flexible, ensure accuracy, and meet changing priorities in a fast-paced, high workload environment. Basic computer literacy to comply with department needs and expectations (i.e., E-HR documentation, obtaining background information and reports, following up on appointments, etc.). Bilingual English/Spanish preferred.

In the spirit of pay transparency, we are excited to share the base range for this position, exclusive of fringe benefits. $33.00 - $41.43
If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and/or experience. In addition to these factors - we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth.
