Overview
The Vermont Agency of Human Services is seeking a Reentry Care Coordination Program Manager to lead a statewide Medicaid waiver program supporting sentenced individuals transitioning from incarceration back into the community. This role provides operational leadership to state and regional care coordination teams, strengthens partnerships across corrections, healthcare, housing, and community organizations, and ensures services are trauma-informed, person-centered, and aligned with state and federal priorities. We are looking for a collaborative leader with experience in human services, criminal justice, or healthcare coordination, strong communication and project management skills. Experience with team-based care systems is essential. A master's degree and prior leadership experience are strongly valued. This position, Reentry Care Coordination Program Manager (Job Requisition #53448), is open to all State employees and external applicants. AHS BACKGROUND CHECKS: Candidates must pass any level of background investigation applicable to the position. In accordance with AHS Policy 4.02, Hiring Standards, Vermont and/or national criminal record checks, as well as DMV and adult and child abuse registry checks, as appropriate to the position under recruitment, will be conducted on candidates, with the exception of those who are current classified state employees seeking transfer, promotion or demotion into an AHS classified position or are persons exercising re-employment (RIF) rights. Environmental Factors
Statewide travel required for site visits, meetings, and community engagement. Minimum Qualifications
Bachelor's degree in public administration, Criminal Justice, Social Work, Public Health, or a related field AND three (3) years or more of experience in program coordination, human services, or criminal justice, including supervisory or lead experience. OR Five (5) years or more of experience in program coordination, human services, or criminal justice, including supervisory or lead experience. Preferred Qualifications
Master's degree in social work, Public Administration, Criminal Justice, or a related field. Direct experience working with individuals involved in the justice system. Familiarity with Medicaid and other federal or state-funded human services programs. Experience in cross-sector collaboration (e.g., justice, housing, behavioral health). Lived experience with incarceration or reentry is strongly valued. Total Compensation
