Annual Salary: $70,000 - $75,000

Location: Stuart, FL
Schedule: Full-Time, Exempt

We are seeking a highly skilled and compassionate Registered Nurse to join our leadership team as a Nurse Case Manager and Alternate Director of Nursing. This role combines clinical expertise with operational leadership, offering the opportunity to directly influence patient outcomes and mentor a dedicated care team.

As the Nurse Case Manager/Alt. DON, you will design individualized care plans, oversee nursing and aide staff, and ensure delivery of exceptional in-home care. You will balance field-based engagement with staff and clients alongside administrative leadership, including case management, staff training, and regulatory compliance. This position is ideal for a motivated RN who thrives in a collaborative environment and is committed to advancing both patient care and professional standards.

What You Will Do



  • Manage client cases from intake through ongoing care, including authorizations, communication with referral sources, and coordination of all skilled and non-skilled disciplines
  • Supervise daily clinical operations and support office functions
  • Conduct start-of-care assessments, supervisory visits, and clinical oversight in client homes
  • Provide training, mentoring, and performance support for nursing staff and aides
  • Ensure compliance with agency policies, professional nursing standards, and state and federal regulations


What You Bring



  • Current RN license in the state of Florida
  • Minimum one year of home care experience preferred
  • Ability to travel to client homes as required, with reliable transportation and a valid driver's license
  • Strong time management skills with the ability to prioritize and organize effectively
  • Excellent communication skills, customer service orientation, and leadership presence
  • Experience with AlayaCare is a plus
