Posting Date 08/18/2025 1305 Utica Ave,Brooklyn,New York,11203-5911,United States of America The Patient Education Programs Educator will be instrumental in helping to drive volume of CKD participants educated to the Kidney Smart program by conducting direct education virtually, support with core operations for the program and provide increased capacity for our field teammates across the country to increase the number of participants receiving lifechanging CKD education through Kidney Smart. Provide telephonic call support to CKD participants in focused areas across the country including education, scheduling, rescheduling, and targeted Spanish support. Review, educate and answer questions on all modality options for dialysis including transplant, hemodialysis, PD and HHD. Provide local coverage support with opportunities and data entry as needed. Complete data entry & tasking into data management system. Other duties as assigned. This position will be open for a minimum of three days. The Wage Range for the role is $23.00-$36.30 per hour. If a candidate is hired, they will be paid at least the minimum wage according to their geographical jurisdiction and the exemption status for the position.
