Overview

**This position covers the Winter Haven Hospital and requires travel to surrounding facilities in the WH area. It is full time, M-Friday, 9am-6pm.

As a Hospice Case Manager, you will have a special opportunity to provide, coordinate and direct the provision of hospice nursing and palliative care to terminally ill patients at Winter Haven facilities and potentially in their home or wherever they call home.

Orientation, Onboarding & Training

You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an R.N is 7-9 weeks, however, is individualized according to your needs and experience.

Responsibilities

  • Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs.

  • Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team.
  • Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician.
  • Functions as a liaison between disciplines and facilities during the admission process.
  • Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate.
  • Communicates significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required.
  • Performs on-call responsibilities and on-call services to patients/families as assigned.
  • Performs other duties as assigned.

Qualifications

  • Current RN license, specific to the state(s) you are assigned to work.

  • One year of clinical RN acute care experience at bedside, required.
  • Current CPR certification.
