POSITION SUMMARY:

Boston Medical Center Health System is a ~$7B system whose mission is to provide Exceptional Care without Exception. The Health System includes a tertiary academic medical center a network of community health centers, a health plan and an affiliated accountable care organization.

In 2021 BMC launched the "Health Equity Accelerator" with the purpose of 'transforming healthcare to deliver health justice and well-being'. The Accelerator, in partnership with Population Health, is developing an innovative multi-disciplinary approach that combines clinical operations, community engagement, health-related social needs programs, and research assets to address racial health inequities.

The Community Wellness Advocate (CWA) Manager is responsible for leadership, supervision, and professional development support of the program's CWA team. The CWA Manager also partners with other program leaders in building strong relationships with community agencies and resources that support the social determinants of health the program patients. The CWA Manager will partner with the Community Health Equity Manager in identifying and developing programming to offer patients throughout the program around economic mobility and nutrition security. As an expert in the CWA role, the CWA Manager will contribute to discussions, decisions, and the development of initiatives to improve the program.

The CWA Manager will play a critical part in population health management, patient navigation, and daily operational support for the CWA team, contributing to the overall effectiveness of our program. This role requires strong communication skills, emotional intelligence, and a commitment to advancing health equity.

ESSENTIAL RESPONSIBILITIES / DUTIES:

Community Wellness Advocate team oversight


  • Supervise a cohort of CWAs, including systematically reviewing their patient panels and the quality of their work and documentation.
  • Provide feedback, coaching, and conducting formal performance appraisals.
  • Responsible for recruitment and onboarding of CWAs.
  • Serves as a subject matter expert in health-related social needs navigation.
  • Provides backup support for the CWA team (in case of staffing capacity) including, outreach phone calls, patient navigation, and program enrollment.
  • Provides advocacy, patient education, and successful warm hand-offs in accessing community-based programs and coordinates long-term support beyond the program.
  • Escalates and presents patients at case review meetings succinctly and logically.
  • Consults with program clinical team on complex patient situations, demonstrating an understanding of how to solicit and incorporate provider feedback in order to continuously develop the most optimal plan for care.

Community programming and support


  • Attend clinic sessions and/or group programming to provide patient escalation support, build relationships with program patients, and identify areas for support and program improvement.
  • Support program clinical operations (including workflow development, EHR navigation).
  • Co-facilitates CWA coffee hours with Community Health Equity Manager.
  • Participates with other staff in activities that include community outreach, presentations to community organizations, development of materials, and phone calls.
  • In collaboration with other program leaders, identifies CWA learning needs and work to meet educational needs of CWAs through educational programming, mentoring, and coaching.
  • Partner with the Community Health Equity Manager to identify and develop community-based programming around economic mobility and nutrition security

NOTE: The CWA will not provide hands on care or other services noted as home health services, including but not limited to performance assessments, provision of care, treatment, or counseling; and/or monitoring of patient's health status.

JOB REQUIREMENTS

REQUIRED EDUCATION AND EXPERIENCE:

HS Diploma with community experience

Bachelor's degree preferred

PREFERRED EDUCATION AND EXPERIENCE

Minimum of 5 years prior healthcare, public health, or community-based experience in community setting.

At least one year of leadership experience in similar setting.

Experience with people management (preferred).

CERTIFICATIONS, LICENSES, REGISTRATIONS REQUIRED

Driver's license

CWA certification preferred

KNOWLEDGE, SKILLS & ABILITIES (KSAs)

  • Strong interest in social determinants of health and advancing racial health equity.

  • Knowledge of healthcare systems and community resources

  • Outstanding interpersonal skills and ability to communicate in a courteous, pleasant and professional manner with direct reports, patients and families, general public, staff members, external agencies, and other organizational leaders.

  • Multilingual skills in languages appropriate to the patient populations served by the medical center preferred (Spanish or Haitian Creole).

  • Outstanding ability to communicate and collaborate with other supervisors and managers from other disciplines to solve problems together.

  • Ability to give direct feedback and regular coaching to improve performance; ability to mentor, guide and motivate others through demonstration of best practices and leading by example.

  • Extensive experience in community health and outreach.

  • Ability to work independently and to make decisions based on established policies and procedures.

  • Demonstrated familiarity with community health and community health worker (CHW) workforce and landscape.

  • Demonstrated ability and experience in areas including program development/management, training, team building, problem solving, public policy and advocacy and community networking.

  • Strong education and training skills.

  • Exceptional organizational and time management skills; ability to multi-task and work independently and as part of a team.

  • Excellent oral and written English communication skills.

  • Understanding of how language, culture and socioeconomic circumstances affect health.

  • Desire to work with diverse, multi-cultural and multi-lingual populations.

  • Proficiency with Microsoft Office applications (i.e. MS Word, Excel, Access, Outlook) and web browsers. Proficiency with data entry and data tracking.

  • Ability to provide leadership and coordinate efforts with other team members

  • Demonstrated leadership abilities, prudent judgement, and professional presence and demeanor.

  • Creative problem solver.

  • Ability to adapt to changes in healthcare delivery at local and systems level.

  • Physical ability to meet the core job responsibilities in accordance with practice setting demands.
