Job Description Summary:

Collaborates with patients, families, and interdisciplinary care teams to provide optimal pharmaceutical care in the transition from inpatient to outpatient/home, including to the next provider of care. Supports the inpatient and outpatient pharmacies by offering the continuity of pharmaceutical care and customer service for children seeking care at Nationwide Children's.

Job Description:

Essential Functions:
  • Practices sound pharmacoeconomic principles in the treatment selection process by evaluating health care coverage, formulary considerations, and ability to pay when applicable. Documents clinical and financial outcomes.
  • Identifies potential adverse drug reactions, drug-drug, drug-disease and drug-food interactions, and incompatibilities affecting patient outcomes.
  • Participates on hospital and/or department committees as related to medication safety and/or quality improvement initiatives while demonstrating pharmacy practice leadership.
  • Determines appropriateness of medication prescriptions and contacts prescribing practitioner if in doubt of appropriateness of order.
  • Processes medication prescriptions to assure timely and accurate dispensing in a form appropriate for the patient.
  • Dispenses to and counsels caregivers on the medication. Manages dispensing information through the patient's profile, reviews patient's profile before dispensing medication to avoid adverse events.

Education Requirement:
  • Bachelor's Degree from an accredited college of pharmacy, required.
  • Post graduate year 1 (PGY-1) or equivalent experience, required.
  • Doctor of Pharmacy, preferred.

Licensure Requirement:

Licensure, or licensure-eligibility, to practice pharmacy in the state of Ohio, required.


Certifications:

Board certification, preferred.


Skills:
  • Clinical competency as evaluated by hiring manager through an independent self-case and/or case presentation in pharmacy.
  • Basic skills in using and documenting in an electronic medical record.
  • Strong communication and interpersonal skills.
  • Aptitude for detail and medication safety skills.

Experience:

Three years of pediatric ambulatory or community care practice experience with responsibility for direct clinical patient care, preferred.
