Provide education about asthma and self-management to patients with asthma, their families, and caregivers and support systems. Provide education about the asthma disease process, exercise, medications, complications and their prevention, goal setting, psychosocial adjustment, and other related topics. Conduct home visits to reinforce family-patient asthma management skills. Implement an asthma education plan for patients. Develop and conduct group teaching sessions. Qualifications: High school diploma or GED. Experience in community-based agency, or community health organization preferred. Ability to interact with patients and families of various cultural backgrounds. Bilingual verbal and written communication skills required (Spanish/English) Valid Wisconsin driver's license, use of an automobile, and current automobile insurance.
