This position will work in concert with the Trauma Program Manager, Trauma Medical Director, Associate Trauma Medical Director, PI Coordinator(s), and Trauma Injury Prevention and Outreach Coordinator(s) to ensure optimal clinical care for the injured patient from admission through discharge utilizing an organized multidisciplinary approach for the delivery of care to trauma patients at the Level 1 Jon Michael Moore Trauma Center (JMMTC). The scope includes maintaining compliance with the requirements for trauma verification at all levels by the American College of Surgeons and West Virginia State Designation of a Level I Trauma Center. Participates in the development of and maintaining an effective trauma performance improvement program through concurrent data collection and systems reviews by attending daily rounds with the trauma service team. The Trauma Nurse Care Coordinator will work closely with the trauma system staff to meet the needs of a Level I Trauma Center. This clinical will interact with patients, family, visitors, physicians, nurses, and allied health professionals within the hospital. The Trauma Nurse Care Coordinator will also participate in additional correspondences as necessary to promote optimal care of the traumatically injured patients and function as a resource/educator for the various multidisciplinary team members. Participates in hospital related and other community activities that promotes injury prevention, safety, and the well-being of the community. As the coordinator of care, treats all internal and external physicians, nursing staff, other healthcare team members, and patients/families as valued customers striving to provide advocacy and service.
MINIMUM QUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Bachelors of Science in Nursing 2. Current unencumbered licensure with the WV Board of Registered Professional Nurses, or appropriate state board where services will be provided, as a registered professional nurse OR Current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC). 3. Obtain certification in AHA Basic Life Support within 30 days of hire. EXPERIENCE: 1. Minimum of three (3) years nursing experience. PREFERREDQUALIFICATIONS: EDUCATION, CERTIFICATION, AND/OR LICENSURE: 1. Certified Emergency Nurse (CEN), Certified Flight Registered Nurse (CFRN), Certified Pediatric Emergency Nurse (CPEN), Certified Transport Registered Nurse (CTRN), Trauma Certified Registered Nurse (TCRN) or equivalent advanced certification. 2. Obtain Trauma Nurse Core Course (TNCC) certification within 6 months of hire and TNCC Instructor certification within 1 year of hire. EXPERIENCE: 1. Knowledge and ability to use various computer programs including the EMR (EPIC), Microsoft programs such as Word, Publisher, Excel, Teams, data base management, and report writers. 2. Nursing experience in emergency services, critical care, or trauma specialty related care area preferred. CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. Assists with coordination of all aspects of patient care for the trauma patients that are admitted to Trauma and Geri-Trauma services (such as monitoring protocol guideline compliance, educating the patients/families and staff, scheduling testing and follow-up, assisting with discharge planning and referrals) by acting as a liaison between the patients, physicians, and the various facilities necessary to transition the patient through the healthcare system as needed. 2. Promotes effective communication and collaboration among the multidisciplinary healthcare team and addresses inter- and intra- departmental issues and initiates action to manage the issues with the appropriate team members and department(s) leadership to include but not limited to investigation, education, and follow-up. 3. Provides and/or coordinates education to patients/families/caregivers utilizing the preferred teaching/learning methods as appropriate and assists with monitoring for effectiveness of teaching and evidence of learning. Also assisting in developing new or updating patient educational materials as needed in conjunction with the bedside staff as applicable. 4. Assists with chart audits and reviews of trauma and non-surgical admissions as needed. 5. Assists the Trauma Coordination team with data collection for performance improvement activities and assists in developing, analyzing, interpreting, and identifying process improvement plans. 6. Through attending daily rounds and while providing patient/family and staff education assists with identification of systems or areas for improvement to promote patient satisfaction. 7. Assists the Trauma Coordination team to update standardized forms, brochures, and other distributed information as needed. 8. Serves as a resource for staff and develops and/or presents educational materials as needed to ensure knowledge/compliance with best practice in the care of the trauma patient. 9. Collaborates with Trauma Program Manager (TPM) and the Trauma Medical Director (TMD), Associate Trauma Medical Director (ATMD), PI Coordinator(s), and Trauma Outreach and Injury Prevention Coordinator(s), Trauma Research Coordinator(s) in the development, implementation, distribution and/or presentation/delivery of outreach and educational programs/activities/symposiums/etc. for both healthcare providers and the community using appropriate educational strategies and materials. 10. Participates in the national trauma performance improvement program (TQIP) by collaborating with all Trauma Coordination team members and other key stakeholders to implement the annual Trauma PIPs Program plan. 11. Leads and/or coordinates Trauma ADHOC Educational committee and works with CEOD and other accrediting agencies as appropriate to provide CEUs/CMEs and trauma credits for all healthcare team members for educational offerings when applicable. 12. Exhibits professional behavior on a consistent basis while representing the JMMTC, demonstrates initiative-taking professional behaviors by assuming responsibility for and complying with continued educational requirements, and identifies own learning needs while seeking learning opportunities both internally and externally. Applies knowledge sharing to professional practice to impact and improve patient care. 13. Assists in the development of trauma guidelines/protocols as well as participates in the updates of the protocols on an annual or bi-annual rotation as directed by the TMD/ATMD. 14. Performs and/or coordinates audits of trauma guidelines/protocols to monitor for compliance and identify the possible need for review/revision or the need for education of the multidisciplinary team members. 15. Coordinate and/or teach established educational programs including but not limited to Trauma Nurse Core Course (TNCC) and Rural Trauma Team Development Course (RTTDC). All transfers, lifts and re-positioning will be completed utilizing the lift equipment and/or other patient handling aids as indicated by the patient's profile and appropriate algorithms. However, the Trauma Clinical Nurse Coordinator should refrain from utilizing any lift equipment for which he/she/they/them were not trained prior to use. 2. Work requires strength and/or stamina, lifting, moving, stooping, reaching, standing, walking, and carrying of materials and equipment weighing 35+lbs as well as sitting for extended periods of time.
