Position Overview

The RN Complex Care Coordinator plays a vital role in helping patients transition smoothly from inpatient care to the community setting. This position involves:




  • Patient Outreach: Quickly establishing rapport over the phone to address post-discharge needs and potential barriers.
  • Assessment and Prioritization: Conducting telephonic assessments, identifying risks, and coordinating care to prevent readmissions.
  • Collaboration: Working closely with providers, case managers, and other care teams to ensure a seamless continuum of care.
  • Problem-Solving: Handling fluctuating patient needs and high call volumes with adaptability and critical thinking.



Ideal Candidate Profile



  • Education: Active RN license; BSN preferred.
  • Experience: Previous experience in care coordination, triage, emergency care, or transitional care is highly desirable.
  • Skills: Exceptional communication, strong critical thinking, and a collaborative mindset.
  • Attributes: Compassionate, patient-focused, and adaptable to fast-paced environments.

Sussex Clinic (Primary), supporting patients across the care continuum.
