Summary
Responsible for working closely with individuals and communities to promote and maintain good health. Responsible for providing education, support, and outreach services to community members to help them achieve their health goals. Areas could include exercise, mental health, medication management, nutrition, health care system navigation, substance use and other health behaviors. Works under the direction of a healthcare professional to provide preventive care and health education to members of the community. Does this position require Patient Care? Yes Essential Functions -Conduct community outreach and engage community members to understand their health needs -Provide individual and group health education and coaching to community members -Assist community members in accessing and navigating the healthcare system, including providing referrals to appropriate health services -Provide basic health screenings and assessments to identify health risks and provide appropriate interventions -Assist in the development of health promotion programs and activities to address community health needs -Collect data and maintain accurate records of community health outreach activities and interventions -Attend meetings and trainings related to community health promotion and education This is a full-time, grant-funded position for someone interested in helping patients with both medical and social problems to improve their access and utilization of healthcare services. A Community Health Worker (CHW) is a trusted member of the community who helps patients better access and coordinate their plan of care.We believe that CHWs have the skills and experience to understand what patients are going through and help them address the social and medical problems that lead to poor health. The goal of a community health worker is to assist the most high-risk patients with the tasks of getting psychiatric/medical care, working on health goals (such as arranging care, filling medication prescriptions, planning healthy meals, or finding time to exercise), and to help them deal with the "real-life" issues that keep them from staying healthy. The CHW will work with patients receiving care at Mass General Brigham Emergency Departments.They are integrated into the emergency medicine team and serve as a bridge between the team and patients in the community. As a CHW in our department, you will develop trusting working relationships with your patients and be supported by a team that includes social workers, nurses, and physicians. Education
