Job Title: Community Health Worker (Spanish)

Duration: 4+ months

Working Hours: Monday-Friday, 30-40 hrs/week; occasional evenings & weekends to accommodate community events and activities.

Location: Chesterfield, VA 23834

Job Description:

  • The Chesterfield Health District's Senior Community Health Worker (Sr. CHW) will aim to increase knowledge through education, prevention, and resource navigation for priority populations in a culturally competent manner. This individual will work closely with our health clinics, local partners and businesses including schools, faith-based organizations, and other agencies to improve client health outcomes.


Job Responsibilities:

  • Works frequently within the district's health clinics, referring patients for social needs.
  • Builds and maintains relationships with residents through door-to-door outreach, involvement in community organizations/coalitions, and performs outreach documentation.
  • Builds relationships with local organizations and leaders to determine health needs of the community and to build trust.
  • Participates in the coordination of regular health promotion activities such as health fairs, organizational events, COVID-19 testing/vaccination events, and other local events.
  • Serves as a community liaison for community members around public health programs and community assessments.
  • Participates in community meetings held by local organizations to ensure resident input.
  • Promotes, facilitates, and coordinates REVIVE! trainings (how to respond to an opioid overdose) for Client persons.
  • Assists with grant application preparation and reporting, as requested.


Minimum Qualifications:

  • English/Spanish bilingualism is required.
  • Bachelor's degree from an accredited college, or equivalent training & experience.
  • At least one year of professional experience in health education, human services, healthcare, or related field.
  • Strong organizational, written communication, and presentation skills.
  • Demonstrated ability to work both independently and collaboratively in teams.
  • Demonstrated experience working with Microsoft Office (Word, Excel, and PowerPoint.), and Google suite applications (Gmail, Google Drive, Google Docs, etc.).
  • Possess and maintain a valid State driver's license.
  • Ability to drive State owned vehicle and/or personal vehicle to each locality.


Mandatory Qualifications:

  • Certified CHW (or can become certified within the first 3 months of employment).
  • Has completed professional training in CHW (provided by IHPI or VDH) or can within the first 3 months of employment.
  • REVIVE! trained.
