As a Field Based Behavioral Health Care Advocate, you'll wear many hats and work in a variety of environments. Sometimes, you'll interact with members leaving the hospital - possibly with new medications or diagnoses. Or perhaps you'll perform home visits, assisting members with safe, effective transitions from care environments to where they live. You may also act as an intermediary between providers and members - serving in numerous roles, such as educator, evaluator, service coordinator, community resource researcher and more. The result? Fewer hospitalizations, ER visits, costly service gaps, and a less stressed, more effective health care system for us all.

This is a Monday - Friday position. Hours are 8am - 5pm Eastern. You will be in the field approximately 35% of the time primarily in the greater Cleveland area with some travel to Akron. When in the field you will visit members in their homes, community centers, hospitals, providers offices, etc. You will work from home / telecommute when not in the field..

Due to business needs, candidates residing in Cuyahoga and Summit Counties are preferred and encouraged to apply. Applicants in surrounding counties may be considered based on proximity to territories with the highest needs.

Primary Responsibilities:



  • Visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or providers' offices
  • Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
  • Support members with condition education, medication reviews, and connections to resources such as Home Health Aides or Meals on Wheels
  • Assist members with the transition from a care facility back to their home

Required Qualifications:



  • Independently licensed clinician, with a Master's degree in Psychology, Social Work, Counseling, or related behavioral health discipline, OR an RN with 2+ years of experience in behavioral health
  • Independent, active, and unrestricted license in the state of Ohio
  • 2+ years of post-licensure experience in a related mental health environment
  • Proficient computer skills and good working knowledge of Microsoft Word and Outlook
  • Access to high-speed internet and a dedicated, distraction-free workspace at home
  • Reliable transportation and the ability to travel within Summit County, Cuyahoga County, and surrounding areas
  • Valid drivers license
  • Reside in or near Summit or Cuyahoga County, Ohio



Preferred Qualifications:



  • 1+ years of experience working with Medicaid population
  • 1+ years of Community Mental Health experience
  • Experience working in an environment that required coordination of benefits and utilization of multiple groups and resources for patients
  • Experience working with low-income populations
  • Experience working with either Medicaid or Medicare funded programs
  • Medical / Behavioral setting experience (i.e., hospital, managed care organization, or joint medical / behavioral outpatient practice)
  • Dual diagnosis experience with mental health and substance abuse
  • Clinical training experience



The salary range for this role is $59,500 to $116,600 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
