Job Summary

The Department of Pharmaceutical Services provides a full range of services for patients cared for by the Medical Center at UCSF and its providers and payors. These services include, but are not limited to: drug procurement, storage, and security; safe, accurate and timely preparation and distribution of drugs; clinical pharmacy services for patients; development of drug use policies within the Medical Center; drug information services for clinicians and patients; and participation in technology assessment and quality assurance programs. The Department of Pharmaceutical Services is affiliated with the UCSF Department of Clinical Pharmacy, an academic department within the School of Pharmacy, which has primary responsibility for teaching professional courses in the Pharm.D. curriculum, clinical research, and public and clinical service.
Under the Department of Pharmaceutical Services, the [Medication Safety Specialist] is involved in all aspects of the medication use process at all pharmacy locations within the UCSF Health System. They may also be asked to participate in pharmaceutical care in other sectors of the Department of Pharmaceutical Services as needed. The [Medication Safety Specialist] will be part of an interprofessional team consisting of pharmacy leadership, nursing, medical and information systems to help improve the medication use process through collaboration, communication and on-going process improvement initiatives. The [Medication Safety Specialist] is an expert on medication safety and as such is expected to continually observe, initiate, develop and communicate necessary process improvements in the medication use system to department leadership and hospital quality leadership. Additional functions include but are not limited to: coordinating the collection, analysis and development of performance improvement actions plans for medication error and adverse drug reaction reporting programs, coordinating the departmental Quality Analysis and Performance Improvements (QAPI) plan and the Medication Error Reduction Plan (MERP) with pharmacy leadership, and coordinating the Medication Safety subcommittee of the Pharmacy and Therapeutics committee with the Medication Outcomes Center (MOC). The [Medication Safety Specialist] will work in both the inpatient and outpatient setting, and will serve as a resource to the pharmacy leadership team and performance improvement department on matters of medication safety and regulatory compliance.

Please note: An offer will take into consideration the experience of the final candidate AND the current salary level of individuals working at UCSF in a similar role.

For roles covered by a bargaining unit agreement, there will be specific rules about where a new hire would be placed on the range.

Required Qualifications


  • Must have graduated from an accredited school of pharmacy and possess a Doctor of Pharmacy degree (PharmD) or equivalent degree as determined by the National Board of Pharmacy. For graduates in a non-US country or jurisdiction, pharmacists must have earned a Foreign Pharmacy Graduate Examination Committee (FPGEC) Certification as determined by the National Board of Pharmacy
  • Completed an ASHP Accredited Postgraduate Year 1 (PGY1) Pharmacy Residency Program or equivalent practice experience.

Preferred Qualifications


  • Completed an ASHP Accredited Postgraduate Year 2 (PGY2) Administration or Medication Safety Pharmacy Residency Program

License/Certification


  • Must possess a current license in good standing to practice pharmacy in the State of California
  • Current certification for Basic Life Support (BLS) from the American Heart Association.
