JOB PURPOSE:

The Behavioral Health Case Manager is part of a multidisciplinary clinical team who serves as the bridge between clinical services, wellness services, and community resources. This position will work with adults and their families to deliver case management, skills training, and medication education in office setting and/or community-based settings. This position is responsible for conducting routine psycho-social-emotional needs assessments, completing Person-Centered Recovery Plans, assisting adults and their families with accessing community resources, providing rehabilitative services, and initiating crisis intervention when needed. All services are documented in an electronic health record and provided in collaboration with a multidisciplinary recovery team.

Services provided by the person in this position should promote recovery and wellness, community integration, independent living within the community and improved quality of life. A high level of detailed electronic documentation is required and adherence to company and state guidelines is expected. A minimum of 60% of hours worked are required to be direct services.


Qualifications

ESSENTIAL JOB RESPONSIBILITIES:

Collaboratively work with the Director of Behavioral Health and the Housing Stability Teams while participating in the multi-disciplinary team approach to client care.



  • Provides Behavioral Health case management to families experiencing housing instability or who are enrolled in Endeavors Housing Programs and who are seeking behavioral health services.
  • Provides appropriate referrals as necessary.
  • Prepares and maintains confidential case records.
  • Consults with service providers and community partners on resource-related issues.
  • Ensures confidentiality and controls access to sensitive information.
  • Collaborates with clinical team to perform risk management and crisis intervention services as necessary.
  • Develops network of trusted contacts within organizations in which to refer and coordinate care.
  • Participates in multidisciplinary team meetings.
  • Follows care follow-up procedures to ensure veterans and/or their family received the services they were referred to and do not have any follow up needs.
  • Assists families to navigate service providers across multiple systems of care.
  • Provides services via telehealth platform, as required.
  • Perform other duties assigned by DBH and / or Housing Programs leadership or designee
  • Demonstrate exceptional customer service, in everything you do, by placing the child, family, or client

ESSENTIAL QUALIFICATIONS:

EDUCATION: Minimum Qualifications: Bachelor's degree in a human services field; comprehensive knowledge

of mental health diagnoses. Ability to work as a member of a team within a fast-paced, recovery-focused

environment is essential to this position. Must have a valid Texas Driver's License.

Preferred Qualifications: One-year work or practicum/clinical experience in case management, crisis

intervention and/or psychosocial rehabilitative services.

CERTIFICATION: Case Management Certification preferred

EXPERIENCE: 3+ years strong administrative/clerical experience preferred; 2+ social service and/or non-

profit experience preferred.

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the

employer's sole discretion.

LICENSES: Driver's License with clear record.

WORK LOCATION: Hybrid work set up can occur, private, secure home office needs to be in place for any

remote work. At minimum initial work from office (in-person) and then a hybrid in-office or in-home work can

be reviewed, scheduled, and agreed upon to meet the mission.

VEHICLE: Must have daily use of a vehicle without prior notice.

OTHER: Must not pose a direct threat or significant risk of substantial harm to the safety or health of

himself/herself or others. Must be available and willing to work evenings, weekends and holidays as required to meet business needs.
