Temporary Employment Program
Job Summary The anticipated duration of this temporary assignment is approximately two months; possibility of extension. Please note this is part-time 20 hours per week in the Mission district.
The Patient Navigator will be assigned to the Citywide Peer Team. Citywide Case Management is a UCSF/ San Francisco General Hospital (SFGH) Department of Psychiatry outpatient program located at 1263 Mission Street, San Francisco. Its mission is to provide intensive case management services to adults who suffer from severe mental illness, co-occurring substance abuse disorders and many who also have chronic medical issues. The Patient Navigator will provide a range of support services to mentally ill clients including: supports community integration skills; provides individual support; applies personal experience as a consumer of mental health services; serves as a role model of recovery; assist client with seeking outside activities and encourage group participation, advocate for consumers, providing accompaniment when indicated to housing, financial, health and legal resources; manages work time effectively, efficiently and productively; works collaboratively with case managers and Wellness Team to provide first hand perspective of engagement in treatment; acts as a liaison with community programs; uses good problem solving skills; develops activities, programs and resources which support consumers; maintain client's records and documentation according to California, San Francisco City and County requirements. Other responsibilities involve monitoring milieu, facilitating therapeutic groups, coordinate and/or organized client outings, cultural events, and other tasks related to job. Key Responsibilities:
Patient Support and Coordination:
- Provides support during patient stays or clinic visits, including scheduling and linking patients to health system services.
- Anticipates patient care coordination needs and sets expectations by providing a general roadmap.
- Addresses patient grievances at the point of service and identifies appropriate resources.
- Proposes new workflows or methods to improve future patient satisfaction.
- Gathers and documents patient issues, barriers, or concerns through regular patient visits and family communications.
- Prepares reports for management review and planning to enhance patient satisfaction.
Service Provision:
- Identifies and communicates client social, health, and recovery needs to the social worker.
- Utilizes resources that address the well-being of the mind, body, and spirit for clients' needs.
- Facilitates peer support groups and supports community integration goals with the social worker.
- Provides individual supportive counseling and co-leads groups on topics such as harm reduction, dual diagnosis, spirituality, and hygiene care.
- Serves as a role model of recovery and applies knowledge of mental health services and substance abuse/dual disorder systems.
- Provides advocacy, accompaniment, and group organization, including guest presentations.
Record Keeping and Administration:
- Maintains client records per California, City, and County requirements and records data in client charts.
- Meets program productivity standards and assists in processing quality improvement requests.
- Attends staff meetings, in-service training sessions, and grand rounds.
- Identifies program problems and makes recommendations to supervisory or administrative staff.
Service Planning, Consultation, and Liaison:
- Utilizes knowledge of mental health and community resources to assist in planning for client needs.
- Works with social workers to provide liaison work with community organizations and improve client opportunities.
- Coordinates community service activities and collaborates with providers to accommodate client needs.

Department Description UCSF's Temporary Employment Program (TEP) recruits and hires temporary employees for immediate clerical and technical support services to UCSF Departments and various off-campus locations. UCSF departments deploy temporary employees to work on special projects, fill in for regular employees who are on vacation or leave, or to temporarily fill a vacant position during recruitment. Frequently temporary employees become successful candidates for career and limited appointment positions.
Required Qualifications
- Understanding of mental health issues and sensitivity toward mental health clients.
- Ability to follow oral and written instructions accurately.
- Ability to work as part of a team, prioritize tasks, handle multiple responsibilities, and work independently in a high-pressure environment.
- Skills to assess and comprehend patient grievances to quickly locate appropriate resources for assistance.
- Ability to work with clients and staff from diverse cultural and social class backgrounds.
- Strong interpersonal and customer service skills, with the ability to be diplomatic, organized, and remain calm under pressure while applying sound judgment.
- Physical ability to lift 50 lbs and climb stairs.
- Good problem-solving skills.
- Knowledge of computers and software, including Microsoft Excel and Word.
- Demonstrated excellent attendance and reliability.
- Bachelor's degree or an equivalent combination of education, training, and experience
